About The Position
Concierge Home Care is hiring Home Health Registered Nurses (RN). This role offers an opportunity to take control of your schedule and income through a pay-per-visit model, providing 1:1 patient care in the home. The position is located in the West Palm Beach, FL / Palm Beach County area, with territories designed to minimize drive time through clustered scheduling. The role emphasizes flexibility, autonomy, and efficient technology to reduce documentation time, allowing nurses to focus more on patient care.
Requirements
- Active, unencumbered Florida Registered Nurse (RN) license
- Valid driver’s license, reliable transportation, and auto insurance
Nice To Haves
- Home health experience preferred, but not required
Responsibilities
- Perform patient assessments and develop individualized care plans
- Provide skilled nursing care and patient education
- Coordinate care with physicians and interdisciplinary teams
- Monitor patient progress and adjust care plans as needed
- Complete documentation using streamlined, AI-assisted tools
