About The Position
We are seeking a compassionate and experienced Registered Nurse Case Manager to support patients and families throughout their healthcare journey. This role focuses on coordinating patient care, developing individualized treatment plans, facilitating discharge planning, and ensuring safe transitions across the continuum of care. The RN Case Manager collaborates closely with physicians, nurses, leadership teams, and interdisciplinary departments to deliver high-quality, patient-centered care.
Requirements
- Active Registered Nurse (RN) license
- Associate Degree or Certificate in Nursing required
- Minimum 2 years of recent acute care experience within the last 3 years

Responsibilities
- Guide patients and families through program orientation and care processes
- Educate patients regarding rehabilitation philosophy, insurance benefits, discharge criteria, and patient rights
- Coordinate family conferences and interdisciplinary treatment planning meetings
- Provide ongoing communication and emotional support throughout hospitalization
- Develop individualized treatment and recovery plans
- Complete psychosocial assessments and discharge planning evaluations
- Identify barriers to discharge and coordinate appropriate resources
- Ensure smooth and safe patient transitions to appropriate levels of care
- Maintain accurate and timely documentation within the medical record
- Collaborate with physicians, nursing staff, rehabilitation leadership, and interdisciplinary teams
- Promote patient advocacy and continuity of care
