This PRN Day shift opportunity is for a Registered Nurse, Case Manager in the In-Patient Case Management Department at the Baptist Medical Jacksonville - Downtown campus. The role involves care coordination for hospitalized patients, high-risk screenings and assessments, coordinating post-acute care services with documentation, and collaborating with multidisciplinary teams. Requirements
- 1 year case management experience
- Associate's Degree Nursing
- Basic Life Support (BLS)
- Driver's Licensed (DMV)
- Active licensed Registered Nurse (RN)
Responsibilities
- Care coordination for hospitalized patients.

- Coordinating post-acute care services with proper documentation.
