About The Position

We are seeking a Registered Nurse Case Manager to provide care for patients across all acute care departments. The role focuses on case management, discharge planning, and progression of care, supported by a dedicated case management team in a Level II Trauma Center with a Level III NICU and Level II Pediatric Trauma unit. New hires receive 1.5 weeks of training and an EPIC class. Free on-site parking is provided. The required attire is navy blue scrubs or professional clothing.

Requirements

• At least 1.5 years of general nursing experience.
• Prior experience in discharge planning and Acute Care Case Management.
• Valid Registered Nurse (RN) license from the South Dakota Board of Nursing.
• Current BLS certification.

Responsibilities

• Manage a caseload of 16–20 patients, ensuring effective discharge planning and progression of care.
• Collaborate with families to discuss comprehensive plans of care.
• Work closely with Case Management Assistants, Lead Case Managers, Social Work professionals, and Utilization Review RNs.
• Use EPIC for documentation and patient tracking.
