About The Position

Dove Healthcare – West Eau Claire is seeking a Registered Nurse (RN) Case Manager to join our team. This role is ideal for an experienced nurse looking to advance their career by leading, mentoring, and supporting a team while overseeing resident care. The position involves coordinating and overseeing care to ensure quality outcomes, supporting palliative care services, and collaborating with rehabilitation teams to help residents achieve their highest level of function. The RN Case Manager will supervise daily care and team member performance, ensure care plans are current and compliant, and assist with staffing coordination. A key aspect of the role is fostering a safe, comfortable, and home-like environment for residents. The position includes participation in an on-call rotation.

Requirements

Current Wisconsin RN license
Graduate of an accredited nursing program
Previous experience in a related setting
Strong leadership, organization, and problem-solving skills
Ability to manage priorities in a fast-paced environment
Team-oriented with excellent communication skills
Passion for providing high-quality, compassionate care

Responsibilities
