Roze Room is a leading provider of Hospice and Palliative Care, celebrating over 25 years of service to Southern California communities. This full-time, field-based Hospice RN Case Manager (RNCM) role in the San Fernando Valley focuses on guiding patients and families through life's vulnerable transitions, delivering comfort, dignity, and compassionate care at home. The role emphasizes making a meaningful impact by leading and coordinating individualized hospice care plans, providing skilled nursing care including pain and symptom management, supervising LVNs and CHHAs, collaborating with the interdisciplinary team, and educating/supporting patients and families.
Requirements
- Licensed Registered Nurse (RN)

Responsibilities
