The Hospice Registered Nurse Case Manager role involves conducting comprehensive patient assessments, including vital signs and medical history review. This position requires travel to patients' residences to conduct visits and develop individualized care plans in collaboration with multidisciplinary teams. The RNCM will coordinate hospice care by working with other team members and patient family members to ensure needs are met. Responsibilities include performing evaluations for hospice care, consulting with physicians regarding patient condition changes, and educating patients and families on treatment plans and medication management. The role also requires prompt submission of clinical notes, participation in IDG and case conferences, and performing on-call duties as needed. Additionally, the RNCM will develop and implement the initial plan of care, supervise and evaluate the care given by the hospice aide, and provide care plans to the Hospice Aide and LPN as part of the IDG team. Other duties as assigned are also part of the role.
