The Hospice Registered Nurse Case Manager (RNCM) conducts comprehensive patient assessments, including vital signs and medical history review. This role involves traveling to patients' residences to conduct visits and developing individualized care plans in collaboration with multidisciplinary teams. The RNCM coordinates hospice care by working with other team members and the patient's family to ensure needs are met. Responsibilities include performing evaluations for hospice care, consulting with physicians regarding patient condition changes, and educating patients and families on treatment plans and medication management. The role also requires submitting clinical notes promptly, participating in IDG and case conferences, and performing on-call duties as needed. Additionally, the RNCM conducts initial and ongoing comprehensive assessments, develops and implements care plans, supervises and evaluates hospice aide care, and provides care plans to the Hospice Aide and LPN as part of the IDG team. All other duties as assigned.
