About The Position
At St. Croix Hospice, the Registered Nurse Case Manager provides one-to-one holistic support to patients and families in their homes. The role partners with an interdisciplinary team to develop individualized care plans, guide end-of-life conversations, and promote patient comfort with professionalism and empathy. The position offers meaningful patient connections and autonomy in practice. The position is based in Bolivar, Missouri, is full time, and has a schedule of 8:00 a.m. to 5:00 p.m. It includes on-call expectations and 100% travel.
Requirements
- Associate, bachelor's, or master's degree from an accredited nursing program and Registered Nurse (RN) qualification.
- At least 1 year of recent acute care experience.
- Reliable transportation, a valid driver's license, and proof of auto insurance.

- Hospice or home health experience.
Responsibilities
- Complete initial and ongoing comprehensive assessments to determine hospice needs.
- Develop individualized care plans with patients and families and update them as needs evolve.
- Administer prescribed medications and treatments.
- Educate patients and caregivers on care techniques, medications, diet, and symptom management.
- Rotate on-call duties and ensure seamless care transitions.
- Partner with an interdisciplinary team to support patients and families and guide end-of-life conversations.
