About The Position

As a Registered Nurse Case Manager (RNCM), you will make a difference in the lives of our patients by providing one-on-one patient care in their preferred environment. You will utilize your leadership skills in coordinating care and provide home-based nursing care for patients as directed by an attending physician. You'll be part of an interdisciplinary team focused on compassionate quality care and positive patient outcomes. This role involves interacting with patients' families and being part of their recovery journey, offering a real-world perspective on restoring health.

Requirements

  • Registered Nurse with current license in the state of employment.
  • Minimum one-year experience as an RN in an acute care setting.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.

Responsibilities

  • Assume ultimate accountability and leadership for the assessment of patient condition and plan of care.
  • Case manage and provide clinical care under the direction of the physician.
  • Educate patients and their family members.
  • Ensure the safety of the patient.
  • Observe and monitor patient conditions and perform OASIS assessments to develop and adjust individualized care plans.
  • Administer medication as prescribed by the physician.
  • Help decrease re-hospitalizations by prioritizing visits for high-risk patients.
  • Oversee and supervise total care of patients provided by nurse aides and LPNs.
  • Manage multi-disciplinary care, promoting continuity of care with appropriate admissions, transfers, and discharges.
  • Educate the patient and family on the disease process using teach-back methods.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Collaborate with the interdisciplinary team and physicians to ensure optimal care.
  • Understand and follow agency policies, procedures, rules, and regulations.
  • Communicate changes in schedule/availability to schedulers or supervisors.
  • Attend in-service training and mandatory agency meetings.
