About The Position
The RN Case Manager coordinates patient care, manages utilization review, and ensures effective discharge planning in an acute care hospital setting.
Requirements
- Active California RN License – Required
- Acute care experience ( MS/Tele/ICU + some ER exposure ) – Required
- California Case Management experience – Mandatory
- Strong experience in Care Coordination & Utilization Review
- InterQual & Epic EMR experience – Required
- Excellent clinical, communication, and documentation skills
Responsibilities
- Coordinate patient care across the continuum
- Perform utilization review using InterQual
- Develop and execute discharge planning
- Collaborate with interdisciplinary teams
- Ensure appropriate resource utilization
- Document in Epic EMR
- Support safe and timely patient discharge
