Requirements

  • Licensed Registered Nurse credentialed from the Kansas Board of Nursing obtained prior to hire date or job transfer date required.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

  • Case Manager credentialed from the American Case Management Association (ACMA) preferred.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
  • Experience in Case Management, Discharge Planning, or Utilization Review.
  • Inpatient clinical experience.

Responsibilities

  • Create comprehensive care plans that span the entire continuum, ensuring that a patient’s unique values and family preferences are at the heart of every clinical decision.
  • Serve as the primary champion for your patients, proactively identifying and removing barriers to care and securing the essential resources they need to thrive.
  • Act as a vital liaison, coordinating between the hospital and post-acute services, ensuring that the hand-off is safe, clear, and supportive.
  • Use your expertise to guide medical record documentation and provide level-of-care recommendations that align treatment options with the best possible outcomes.
  • Advocate for the efficient and effective use of clinical resources, balancing high-quality care with smart operational stewardship.
