About The Position

At St. Croix Hospice, the Registered Nurse Case Manager serves as a cornerstone of patient care, delivering individualized, holistic support to patients in their homes. The role collaborates with a care team to develop personalized plans of care, leads end-of-life conversations, and ensures symptom management with empathy and clinical excellence. The position is based in Harrisonville, Missouri, includes 100% travel, and is full-time with on-call expectations. The stated schedule is 8:00 a.m. to 5:00 p.m.

Requirements

• Associate, bachelor's, or master's degree from an accredited nursing program (RN).
• At least 1 year of recent acute care experience.
• Reliable transportation, a valid driver's license, and proof of auto insurance.

Nice To Haves

• Hospice or home health experience preferred.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families, updating them as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Collaborate with the care team to provide individualized, holistic patient support.
• Lead end-of-life conversations and support exceptional symptom management.
