About The Position
The RN Case Manager coordinates patient care across the continuum, ensuring effective resource utilization and safe, timely discharge planning in an acute care setting.
Requirements
- Active Alaska RN License – Required
- Minimum 1+ year Case Management experience – Required
- Acute care experience – Required
- Epic EMR experience – Required
- BLS Certification – Required
- Strong clinical, communication, and organizational skills
Nice To Haves
- ACM Certification
- Experience in Magnet / Level II Trauma facilities
- Utilization review or discharge planning experience
Responsibilities
- Coordinate care plans with multidisciplinary teams
- Perform utilization review and manage resource allocation
- Develop and implement discharge planning
- Communicate with patients, families, and providers
- Ensure continuity of care from admission to discharge
- Document in Epic EMR
- Identify and resolve care/discharge barriers
- Support quality and compliance initiatives
