About The Position

Baptist Health is seeking a Registered Nurse, Case Manager to join the In-Patient Case Management Department at Baptist Medical Jacksonville - Downtown campus. This is a Full Time Day shift opportunity, working Monday - Friday. As a Case Manager, you will be responsible for care coordination for hospitalized patients, high-risk screenings and assessments, coordinating post-acute care services with proper documentation, and collaborating with multidisciplinary team members.

Requirements

• 1 year case management
• Associate's Degree Nursing
• Basic Life Support (BLS)
• Driver's Licensed (DMV)
• Active licensed Registered Nurse (RN)

Responsibilities
