About The Position

Dove Healthcare is seeking a Registered Nurse Case Manager to coordinate and oversee patient care while mentoring, training, and educating a team of caregivers. The role works with employees, patients, family members, and other professionals to coordinate person-centered care. This is a limited-term, full-time position with the potential to become permanent. The position participates in an on-call rotation. Location: Dove Healthcare - St. Croix Falls, 750 E Louisiana Street, St. Croix Falls, WI 54024. A $5,000 sign-on bonus is offered.

Requirements

• Current licensure in the State of Wisconsin as a registered nurse (RN).
• Degree from an accredited school of nursing.
• Previous nursing experience.

Responsibilities

• Supervise the day-to-day activities of assigned residents and employees.
• Coordinate, plan, implement, and evaluate nursing services provided to each resident using clinical competence.
• Review and audit compliance with the patient's plan of care and changing needs.
• Provide a safe, comfortable, and home-like environment.
• Participate in an on-call rotation.
• Mentor, train, and educate a team of caregivers.
• Coordinate aspects of person-centered care with employees, patients, family members, and other professionals.
