About The Position

The Registered Nurse - Case Management position at UAB St. Vincent's Birmingham is a full-time role focused on coordinating the overall interdisciplinary plan of care for patients from admission to discharge. This involves creating a plan for care across the continuum, integrating patient and family preferences, and monitoring medical necessity and level of care. The role also includes advocating for resources, maintaining communication with supervisors and the care transition team, and serving as a resource for medical record documentation and post-discharge care planning.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required.
Diploma from an accredited school/college of nursing and professional licensure at time of hire required.

Responsibilities

Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.
Creates plan for care across the continuum, integrating patient/family preferences and values.
Monitors patient medical necessity and level of care through assessments, ongoing evaluations and/or patient records.
Advocates for resources and removal of barriers.
Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
Acts as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.
