About The Position

The Registered Nurse - Case Management is responsible for coordinating the overall interdisciplinary plan of care for patients from admission to discharge. This role involves creating a plan for care across the continuum, integrating patient and family preferences and values. The nurse will monitor patient medical necessity and level of care through assessments, ongoing evaluations, and patient records. They will advocate for resources and the removal of barriers, maintaining ongoing communication with their supervisor and care transition team members to ensure effective implementation and reevaluation of the health plan. Additionally, the nurse will serve as a resource for adequate medical record documentation, level of care recommendations, and services related to diagnoses and treatment options for post-discharge care.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required.
Diploma from an accredited school/college of nursing and professional licensure at time of hire required.

Nice To Haves

Rotating weekends every 8 weeks, and holiday rotation.
