About The Position
A geriatric clinic case manager coordinates care for older adults to ensure their medical, social and emotional needs are met. The case manager will support caregivers by providing resources and education to support care of dementia patients.
Requirements
- Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
- Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
Nice To Haves
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
Responsibilities
- Evaluate a patient’s physical, cognitive, emotional, and social functioning to identify deficits and possible needs before the first consultation appointment.
- Develop adaptable, short- and long-term care plans, and regularly check in to adjust these plans as the patient's health and caregiver's need evolves.
- Help families understand and access complex systems.
- Ensure the patient's and caregiver's preferences are respected, advocate for their dignity, and educate caregivers on how to provide appropriate support.
