About The Position

This role involves coordinating the comprehensive, interdisciplinary plan of care from admission to discharge, integrating patient and family preferences to ensure seamless transitions across the care continuum. The nurse will monitor patient progress through continuous assessments and medical record reviews, actively advocating for necessary resources and the removal of barriers to care. They will maintain ongoing dialogue with supervisors, physicians, and healthcare providers to ensure the effective implementation of the health plan and alignment of treatment goals. Additionally, the nurse will act as a key clinical resource for ensuring precise medical record documentation, verifying the appropriateness of services relative to diagnoses, and coordinating optimal post-discharge care options.

Requirements

  • Licensed Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
  • Licensed as a Registered Nurse in Florida required.
  • Credentialed from State Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

  • BLS Provider preferred.
  • Acute care case management experience
