About The Position

The Registered Nurse Care Manager is accountable for proactively coordinating assigned patients’ care and ensuring timely transitions to the most appropriate level of care across the continuum. The role contributes to performance in length of stay, cost-efficient resource utilization, and prevention of readmissions and unnecessary emergency room visits. The Care Manager works collaboratively with physicians, nursing staff, the multidisciplinary team—including Home Care and PCP offices—and other internal and external resources.

The position is in Case Management and works the day shift, 8:00 a.m. to 4:30 p.m., for 80 scheduled hours per biweekly pay period.

Requirements

• State licensure as a Registered Nurse (RN).
• Bachelor’s degree in nursing from an accredited educational institution, or actively pursuing the degree with completion required within five years of accepting the position.
• Three years of acute hospital care experience.

Nice To Haves

• Experience in utilization management/case management, critical care, or patient outcomes/quality management.
• Case management certification (ACM or CCM).
• Basic Life Support (BLS) certification as a Healthcare Provider from the American Heart Association, American Red Cross, or equivalent through the Military Training Network (MTN).

Responsibilities

• Perform care coordination assessments, including initial assessments within 24 hours of admission, readmission assessments, and transition planning.
• Collaborate with the social worker and other disciplines to ensure a safe, appropriate, and timely transition to the next level of care, considering the patient’s available resources.
• Assess patient and family needs to reduce barriers and formulate discharge plans, including addressing length-of-stay barriers to discharge.
