Requirements

• Active WA State Registered Nurse (RN) license required.
• Active Basic Life Support (BLS) for Healthcare Providers required.
• Active specialty certification in care management within twelve (12) months of hire required.

Nice To Haves

• Completion of a BSN, or an ADN from an accredited institution.
• Two or more years of ambulatory nursing experience is highly preferred.
• BSN or three years with an ADN is preferred.
• Experience in areas such as public health, community health, or equivalent.
• Experience in multicultural nursing or other social service area and commitment to community health care is preferred.
• Care management or chronic disease management experience is highly preferred.
• Certified Diabetes Educator (CDE) preferred.

Responsibilities

• Plans, organizes, and provides nursing services to clinic patients in conjunction with clinic providers.
• Works collaboratively with providers and other members of the health care team to improve the health of patients with chronic disease conditions and/or complex needs.
• Educates patients and families to help them manage their health care needs.
• Facilitates communication, coordinates services, addresses barriers, and promotes optimal allocation of resources while balancing clinical quality and cost management.
