About The Position

Loyola Medicine is seeking a Registered Nurse Care Coordinator for Population Health to provide exceptional, person-centered care. The role focuses on treating the whole person—physically, emotionally, and spiritually—and coordinating care so patients receive appropriate services in the right setting and at the right time. The Care Coordinator supports wellness and care goals, helps prevent illness, improves clinical outcomes and quality of life, and assists patients managing multiple chronic conditions. The position uses current nursing practices and contributes to a healthcare team by developing and providing comprehensive plans of care for assigned patients. It also supports patients with complex chronic medical and psychosocial needs and reflects service excellence through care, concern, respect, and cooperation.

Requirements

Bachelor’s degree.
6–10 years of previous job-related experience, including 3–5 years in sub-acute, acute inpatient rehabilitation, or a similar setting.
Current Registered Nurse license in the State of Illinois.

Nice To Haves

Master’s degree.
National certification in case management.

Responsibilities

Use current nursing practices to provide patient care focused on patient needs and service excellence.
As a contributing member of the healthcare team, develop and provide comprehensive plans of care for assigned patients.
