About The Position

Colorado Visiting Nurse Association has an immediate opening for a Registered Nurse - CAPABLE. This is a part-time, PRN position with per visit rates. The role involves providing client care using registered nursing skills and processes in accordance with applicable scope and standards of practice, keeping within the policies, values, and mission of the organization. Upon completion of required training, the nurse will conduct semi-structured in-home client interviews to identify and prioritize problem areas, provide initial and ongoing assessments, and implement strategies including Motivational Interviewing techniques. The nurse will capture participant goals in up to 3 Action Plans, engage clients in problem-solving, coach and guide individuals in building self-efficacy, reinforce brainstorming techniques, and address education and resources for future needs. Collaboration with other disciplines, such as the CAPABLE OT and primary care providers, is essential. Participation in community education, team training, staff/board committees, and employee orientation is also expected.

Requirements

  • Graduate of an accredited school of nursing; BSN preferred.
  • One to two (1-2) years of experience in hospice and/or palliative care, medical/surgical, public health, oncology, acute care or home health.
  • Current Colorado RN license.
  • Valid Colorado state driver’s license and proof of auto insurance.
  • Current CPR certification.

Responsibilities

  • Provides client care using registered nursing skills and processes in accordance with applicable scope and standards of practice, keeping within the policies, values, and mission of the organization.
  • Upon completion of required training through the CAPABLE National Center, conducts a semi-structured in-home client interview to identify and prioritize problem areas.
  • Provides initial and ongoing assessments and implements strategies including Motivational Interviewing techniques to conduct a semi-structured client interview to identify and prioritize problem areas.
  • Capture the goals of the participant in up to 3 Action Plans which are developed and implemented.
  • Engage clients in problem-solving to identify behavior and environmental contributors that may impact function.
  • Coach and guide the individual in building self-efficacy and confidence.
  • Reinforce brainstorming techniques, strategic thinking, and more independent problem-solving to promote the ability to live at home and in the community.
  • Address education and resources to address future needs.
  • Collaborates across disciplines involved with care of the client, including but not limited to the CAPABLE OT and primary care providers as needed.
  • Participates in related services, such as community education, team training, staff/board committees and orientation of employees.
