CBI Home Health is looking to hire Registered Nurse/Registered Practical Nurse in Your Care At Home. Join the YourCare@Home program, where CBI Home Health is supporting the Ottawa Hospital by providing outstanding, person-centered care to help patients achieve the best possible outcomes as they return home from the hospital. As part of an integrated team, CBI Home Health team members work closely together to restore function, independence, confidence, and social connection. CBI Home Health provides training and support so that team members can deliver health care in the home that meets best practice standards and makes a meaningful difference to both patients and to the health care system.

Requirements

  • Active registration with the College of Nurses of Ontario in good standing
  • Professional Liability Insurance (WeRPN/RNAO)
  • A clear criminal record check with vulnerable sector screening results in the past 12 months
  • Proof of negative TB test and professional liability insurance
  • Ability to travel independently within the community
  • A valid driver's license (G2/G) and vehicle insurance are required
  • Strong critical thinking skills
  • Strong verbal and written English communication skills

Nice To Haves

  • Palliative training is considered an asset
  • Pediatric experience is considered an asset
  • Bilingual in French an asset

Responsibilities

  • Apply clinical skills in areas such as wound care, IV therapy, palliative care, pediatric care, tube feeding and medication administration, as well as health teaching.
  • Utilize a person and family centered approach to care to guide decision making and facilitate collaborative care with each member of the interdisciplinary team.
  • Provide mentorship and support for ongoing development and clinical excellence.
  • Support the larger health system by demonstrating confidence in navigating clients and families to other resources to help them meet their goals.
