About The Position

This role is for a Registered Nurse in the Behavioral Health field, specifically within the Adult Psychiatry Unit at Ascension St. Francis Hospital in Milwaukee, WI. The position is on a PRN (as needed) basis, with day shifts available. The nurse will be responsible for implementing and monitoring patient care plans, recording and communicating patient conditions, coordinating care among different disciplines, carrying out physician and nursing orders, and assessing patient discharge planning needs.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted required.
Registered Nurse obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Responsibilities

Implement and monitor patient care plans.
Monitor, record and communicate patient condition as appropriate.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Note and carry out physician and nursing orders.
Assess and coordinate patient's discharge planning needs with members of the healthcare team.
