Plans, provides and/or supervises the nursing care of patients in an efficient and caring manner. Coordinates care planning with other caregivers and with other areas, adheres to universal precautions, and is compliant with infection control policies. Works independently with minimal supervision. Chemical Dependency Counselor (CDC) addendum: Assists the medical director with patient rounds. Provides assessment and therapeutic counseling to chemically dependent clients using a bio-psychosocial approach in an outpatient setting. Educates clients and families regarding the disease process for chemical dependency. Conducts individual and group therapy and leads group discussion. Responsible for the care planning of a caseload of clients with chemical dependency issues. Assesses patient needs and updates medical director.
