Requirements

  • Active Pennsylvania Registered Nurse (RN) license
  • Knowledge of OBRA and Department of Health regulations
  • Strong attention to detail, collaboration, and communication skills

Nice To Haves

  • RNAC experience or MDS/Resident Assessment Certification

Responsibilities

  • Take part in MDS & Care Planning Process: Ensure accurate, timely completion and compliance with federal, state, and facility standards.
  • Collaborating Across Teams: Act as the liaison between nursing and interdisciplinary team members to ensure seamless care coordination.
  • Driving Clinical Excellence: Evaluate Residents for significant changes, initiate assessments, and support Medicare eligibility determinations.
  • Educating & Empowering Staff: Provide ongoing education and guidance on MDS processes and care planning best practices.
  • Ensuring Compliance & Accuracy: Maintain documentation integrity, coordinate submissions, and track Resident status updates with regulatory agencies.
  • Supporting Quality Improvement: Participate in Quality Assurance initiatives, committee work, and ongoing care improvement efforts.
  • Stepping in When It Matters Most: Provide hands-on nursing support when needed to ensure Residents receive the highest level of care.
