About The Position

Meadow View Nursing Center, in Berlin, PA provides a full range of services in our highly regarded program of nursing and rehabilitation and nursing-centered care. We pride ourselves on positive outcomes and personalized services. We are currently seeking an experienced Registered Nurse Assessment Coordinator- (RNAC) to join our collaborative team to support regulatory compliance, resident centered care and survey readiness. Requirements

Current RN license is required
Basic computer knowledge is essential
Strong organizational and communication skills

Nice To Haves

Associate or Bachelor Degree in Nursing preferred.
Previous experience in completion of Resident Minimum Data Sets and Care planning preferred.
RAC-CT certification

Responsibilities

Conduct and coordinate the development and accurate completion of the resident assessment and care plan process in compliance with all federal, state and local guidelines which govern the facility.
Coordinate scheduling of OBRA and Medicare/managed care assessments, entry/re-entry records, and care plan reviews in accordance with the most recent regulatory guidelines.
Inform residents, responsible parties and staff of Medicare/managed care coverage status through appropriate documentation in accordance with all federal, state and facility guidelines. Provides updates as necessary.
Works with Community Health Choice (CHC) case managers for Managed Medicaid providers.
Participate in committees as directed.
