About The Position

We are seeking a compassionate and experienced RNAC (Resident Nurse Assessment Coordinator) to join our team at Simpson Senior Living. The RNAC plays a crucial role in assessing the health needs of our residents and coordinating individualized care plans to ensure the highest quality of care.

Requirements

Valid Registered Nurse (RN) license in Pennsylvania.
Minimum of 3-5 years of nursing experience in a senior living or long-term care setting.
Strong knowledge of Medicare/Medicaid regulations and reimbursement systems.
Excellent communication and interpersonal skills, with the ability to collaborate effectively with interdisciplinary teams.
Detail-oriented and organized, with the ability to prioritize tasks and manage multiple responsibilities effectively.

Nice To Haves

Certification in Resident Assessment Coordinator (RAC-CT) preferred.

Responsibilities

Conduct comprehensive assessments of residents' health status, including physical, mental, and emotional well-being.
Collaborate with interdisciplinary teams, including physicians, nurses, therapists, and social workers, to develop personalized care plans for residents.
Ensure accuracy and compliance with regulatory requirements in resident assessments and care planning documentation.
Coordinate and facilitate care planning meetings to discuss residents' progress, goals, and care plan adjustments.
Provide ongoing education and support to staff members regarding assessment processes, documentation requirements, and best practices in resident care.
