The Registered Nurse (RN) is a vital member of the Assertive Community Treatment (ACT) team, providing rehabilitation, treatment, and crisis intervention under clinical supervision. This role focuses on delivering person-centered care, coordinating treatment activities, and offering supportive interventions to help individuals develop essential skills for independent living. The RN also plays a key role in medication management, assisting psychiatrists with administrative tasks, ensuring accurate medical documentation, and educating both clients and team members on medication use. The ACT team is a multidisciplinary group dedicated to supporting adults with serious mental illness (SMI) through community-based services, including therapy, medication management, and resource linkage. Services are primarily delivered in the community, with clients receiving at least three home visits per week. The RN contributes to a holistic approach to care, helping clients integrate into society while ensuring their basic needs and treatment goals are met. Frequent travel (50-90%) is required, and personal vehicle use for client transportation may be necessary.
