About The Position

Healthcare doesn’t stop when the office closes. Some of the most important decisions happen after hours—when a hospice patient develops new symptoms, a home health nurse needs clinical guidance, a provider requires support, or a family simply needs reassurance. As Riverside’s After-Hours Triage & Clinical Support Nurse, you’ll be the experienced clinical leader helping patients, families, clinicians, and providers navigate those moments with confidence. This is much more than answering phone calls. You’ll serve as the clinical hub for Hospice, and Provider Services, providing expert triage, supporting field clinicians, ensuring quality documentation, and helping our teams deliver exceptional care after hours. As Riverside’s After-Hours Triage & Clinical Support Nurse, you’ll help ensure patients receive the right care at the right time while supporting clinicians across Hospice, and Provider Services. Your work directly impacts patient comfort and safety, family confidence and peace of mind, reduction of unnecessary emergency department visits and hospitalizations, timely symptom management, clinical decision support for field staff, quality documentation and regulatory compliance, patient satisfaction and service recovery, and strong relationships with community partners. Every shift helps hundreds of patients continue receiving exceptional care—even while the rest of the organization is sleeping.

Requirements

  • Active Oregon Registered Nurse (RN) license
  • Graduate of an accredited school of nursing
  • Current CPR certification
  • Minimum of two years of nursing experience
  • Homecare Homebase (HCHB) experience required
  • Excellent clinical assessment and critical-thinking skills
  • Strong communication and documentation abilities
  • Ability to work independently with minimal supervision
  • Excellent organization and time-management skills
  • Comfortable providing clinical guidance to fellow nurses and collaborating with physicians and interdisciplinary teams

Nice To Haves

  • Hospice experience strongly preferred
  • Hospice triage experience strongly preferred

Responsibilities

  • Provide after-hours telephone triage for Hospice, and Provider Services patients
  • Assess patient symptoms and determine the appropriate level of care
  • Provide evidence-based symptom management recommendations
  • Determine when field visits are necessary and coordinate appropriate follow-up
  • Collaborate with providers regarding medications, treatment plans, and physician orders
  • Serve as the primary clinical resource for after-hours field nurses
  • Support on-call RNs and LPNs with clinical questions and complex patient situations
  • Provide real-time guidance regarding symptom management, patient care, and agency processes
  • Escalate complex concerns to leadership when appropriate.
  • Place orders for after-hours medications refill needs and coordinate with the pharmacy.
  • Coordinate after-hours visits with hospice runners and on-call clinicians
  • Process physician orders and scheduling workflows
  • Ensure timely communication with each patient’s care team
  • Maintain continuity of care across Home Health, Hospice, and Provider Services
  • Complete accurate documentation within Homecare Homebase (HCHB)
  • Conduct routine chart audits for quality and regulatory compliance
  • Participate in QAPI initiatives and performance improvement projects
  • Assist with CAHPS and patient satisfaction initiatives
  • Identify opportunities for service recovery and quality improvement
  • Complete welcome calls for newly admitted hospice patients and families
  • Conduct routine patient and caregiver satisfaction calls
  • Build positive relationships with facility partners
