About The Position

This role demonstrates the use of the nursing process as a problem-solving model, utilizing knowledge and experience to anticipate and plan for patient and family needs during admission, transfer, and discharge. The nurse reviews all orders for new admissions, transfers, and discharges, and communicates patient changes and conditions with assigned team members. They communicate with patients/families within 24 to 72 hours of admission and daily with the interdisciplinary team after rounds. Collaboration with the Primary RN and case managers/social services is essential during the patient's stay. The nurse assumes a proactive role in coordinating interdisciplinary discharge planning for specific patient populations and facilitates outpatient resources in collaboration with the interdisciplinary team. Working with NPs and other Providers, the nurse ensures patients are ready for discharge by 12 noon. Commitment to meeting the learning needs of patients and families is demonstrated through the utilization of appropriate resources to achieve positive patient outcomes. The nurse supports patients/families in maximizing their sense of control and active participation in recovery. This includes medication reconciliation, finalizing discharge orders, completing the discharge summary, providing patient/family education, and documenting discharge teaching. Validation of patient/family education and comprehension prior to discharge is crucial. The nurse facilitates all follow-up appointments and ensures ordered tests/procedures are scheduled before discharge. Post-discharge, the nurse communicates with patients/families 24 hours after discharge, documenting the encounter in UChart, and conducts follow-up at 7, 14, and 30 days post-discharge, as needed, also documenting these encounters. The role serves as a liaison between the medical/surgical team for patients/families post-discharge. The nurse demonstrates the knowledge and skills necessary to provide care appropriate to the age and needs of patients served on the assigned unit, serving as a direct care provider as appropriate to ensure positive outcomes. Maintaining a safe and clean environment that promotes patient/family safety throughout all environments of care and complying with regulatory standards, including National Patient Safety Goals, are key responsibilities. Creativity and innovation in approach to complex problem-solving are also expected.

Responsibilities

Demonstrates the use of nursing process as a problem-solving model; utilizing their knowledge and experience to anticipate and plan to meet patient and family needs at time of admission, transfer, and discharge.
Reviews all orders on new admission, transfers, and discharges.
Communicates patients changes and condition with the assigned team members as indicated.
Communicates with patient/family within 24 to 72 hours (about 3 days) of admission.
Communicates daily with the interdisciplinary team after rounds.
Collaborates with Primary RN, case managers/social services during the patient's stay.
Assumes a proactive role in coordinating interdisciplinary discharge planning for a specific patient population and facilitates outpatient resources that the patient may need in collaboration with the interdisciplinary team.
Works with NP (Nurse Practitioner) and other Providers throughout the stay to ensure patient will be ready for discharge by 12 noon.
Demonstrates commitment to meeting the learning needs of patients and families.
Utilizes appropriate resources to assist in meeting those needs and achieve positive patient outcomes.
Assists patient/family to maximize sense of control and actively participate in his/her recovery.
Supports the patient discharge process in terms of medication reconciliation and finalizing discharge orders completes the discharge summary, provides patient/family` education, and documents discharge teaching.

Facilitates all follow-up appointments and ensures ordered tests/procedures are scheduled prior to patients discharge.
