About The Position

BAYADA Home Health Care is seeking compassionate, dedicated, and experienced Admissions Registered Nurses (RNs) to join their Ocean County Home Health team. This role offers the opportunity to provide exceptional one-on-one patient care with the autonomy and flexibility of home health nursing. Requirements

• Current New Jersey Registered Nurse (RN) license in good standing.
• Minimum of one year of nursing experience; home health experience preferred.
• Experience with OASIS documentation and completing Start of Care visits required.
• Comfortable utilizing an EMR; HCHB experience is a plus.
• Excellent assessment, communication, critical thinking, and organizational skills.
• Reliable transportation and willingness to travel throughout Ocean County.

Nice To Haves

• Home health experience preferred.
• HCHB experience is a plus.

Responsibilities

• Complete comprehensive Start of Care (SOC) and admission visits for home health patients.
• Perform thorough patient assessments and accurately complete OASIS documentation.
• Develop individualized plans of care in collaboration with physicians and the interdisciplinary team.
• Deliver skilled nursing care, including medication administration, wound care, treatments, and patient education.
• Monitor patient progress and communicate changes in condition to the care team.
• Educate patients and caregivers to promote independence, safety, and successful outcomes at home.
• Perform approximately 1–3 Start of Care visits per day within a designated territory, allowing for quality one-on-one care.
