Requirements

  • Bachelor of Science in Nursing (BSN) required; candidates with an Associate Degree in Nursing (ASN) who are actively enrolled in a BSN program will also be considered.
  • Current New York State Registered Professional Nurse license required.
  • Current BLS certification from the American Heart Association required.
  • Previous patient care experience in a hospital setting preferred.
  • Strong communication, clinical assessment, prioritization and patient education skills.
  • Proficiency with electronic medical records (EMR, Epic preferred) and computer systems.

Responsibilities

  • Facilitate seamless, timely and patient-centered admissions and discharges.
  • Complete required assessments and admission/discharge documentation.
  • Provide individualized education and support to patients and families.
  • Assess, plan, implement and evaluate nursing care using sound clinical knowledge and judgment.
  • Collaborate with the interdisciplinary team to coordinate care and support safe transitions.
  • Delegate appropriate patient care activities while maintaining responsibility and accountability for nursing care.
  • Respond appropriately to individual patient needs, including age, language, culture, health literacy and developmental or physical needs.
  • Maintain patient safety, infection prevention practices and established standards of care.
  • Maintain professional competency and participate in continuing education relevant to the clinical area.
  • Participate in unit-based performance improvement initiatives and support departmental goals.
