Hiram W. Davis Medical Center, located in Petersburg, Virginia, is a medical facility providing acute medical diagnoses, skilled care, intermediate care, and long-term care to individuals receiving services in DBHDS facilities and within the community that have intellectual and/or developmental disabilities. Although HWDMC provides long-term care services, current admissions are being accepted for temporary stays and special hospitalization only. Reasons for special hospitalization include but are not limited to acute care, step-down from a community hospital to stabilize, and rehabilitation. The Admissions & Discharge Coordinator – LPN carries out the screening process for admissions and coordinates the discharge process for individuals with intellectual or developmental disabilities discharging to community placements. The Admissions & Discharge Coordinator is highly interactive and acts as a critical link between various entities (both inside and outside the organization). This position works with all interdisciplinary team members including physicians, nurses, social workers, therapists, dietitians, MDS Coordinators, and Pharmacists. This position must communicate effectively with DBHDS facilities, Central Office, families, Community Service Boards (CSB’s), and private providers. This position plays an integral role in facilitating discussion in team conferences, ensuring the team conference is organized, and documenting appropriately. The hours of this position are Monday through Friday, typical business hours, but may vary based on the needs of the facility.
