About The Position

The Registered Nurse 2 - Heart Transplant position at UW Medical Center - Montlake provides expert nursing care to post heart transplant patients. This role involves maintaining knowledge of post-heart transplant patient care, interpreting patient data to provide appropriate nursing services, and administering medications correctly. The nurse will provide care based on patient needs, preferences, and available resources, considering principles of growth and development for age-appropriate care. The position requires systematic assessment of patient needs, effective pain management, and accurate documentation, including UNOS and transplant database requirements. Patient care coordination includes formulating and revising individualized care plans, treating patients and families with respect, and addressing their psychological, cultural, ethnic, and religious/spiritual needs. The role also involves functioning as a liaison between administration, patients, physicians, and other healthcare providers. Telephone communication includes providing medical advice, documenting care, and efficient triage for post-heart transplant patients. Patient safety is paramount, requiring compliance with safety and infection control policies and reporting potential harm through the PSN system. Client teaching involves integrating culturally appropriate education tailored to patient and family comprehension. Enabling operations include coordinating care, communicating effectively with the transplant team, demonstrating flexibility and organization under stress, orienting new personnel, serving as a preceptor and mentor, adhering to attendance standards, understanding billing policies, and utilizing various computer applications such as EPIC, Transplant database, UNET, and other UWMC programs. Professional growth is encouraged through teaching, sharing expertise, participating in professional organizations, and staying informed through staff meetings.

Requirements

  • Bachelors of Science Degree in Nursing
  • Active, unrestricted Washington State Registered Nurse License
  • Minimum of 1-2 years of cardiac-related nursing experience in a cardiac ICU and/or Cardiac Telemetry Unit
  • BLS certification required

Nice To Haves

  • 1 year experience as a post-heart transplant nurse coordinator preferred
  • ACLS certification preferred

Responsibilities

  • Provides expert nursing to post heart transplant patients by maintaining knowledge and care of post-heart transplant patients.
  • Interprets data about the patient’s health and transplant status to provide appropriate nursing care.
  • Administers medications correctly based on age and clinical condition, adhering to the five rights.
  • Provides patient care based on acuity/urgency of patient needs, preference, choices, and available resources.
  • Applies knowledge of growth and development over the life span for age-appropriate care.
  • Interprets data about the patient’s status post-transplant to identify age-specific needs and provide necessary care.
  • Systematically assesses the need for and delivery of nursing services.
  • Performs systematic assessments on post heart transplant population.
  • Assesses and reassesses pain, using appropriate pain management techniques and educating patients/families.
  • Meets documentation standards and policies, including UNOS + transplant database.
  • Formulates, coordinates, and revises individualized plans of care.
  • Treats patients and their families with respect and dignity.
  • Identifies and addresses psychological, cultural, ethnic, and religious/spiritual needs of patients and their families.
  • Functions as a liaison between administration, patients, physicians, and other healthcare providers.
  • Provides care for individual patients over the telephone using the nursing process.
  • Follows policies and procedures for providing telephone medical advice to post transplant patients.
  • Documents care provided via telephone per UWMC policy.
  • Provides efficient and effective telephone triage for post heart transplant patient population.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors.
  • Complies with specific UWMC policies for safety and infection control.
  • Utilizes PSN to report events causing or having the potential to cause patient harm.
  • Integrates patient and family education into the delivery of care, ensuring educational activities are culturally appropriate and tailored to comprehension.
  • Formulates a teaching plan based upon identified learning needs and evaluates its effectiveness, including family as appropriate.
  • Coordinates the care given by others to achieve efficient clinic operations.
  • Communicates appropriately and clearly to manager, co-workers, physicians, and all members of the transplant team.
  • Demonstrates ability to be flexible, organized, and function well under stressful situations.
  • Orients new permanent and temporary personnel to the transplant team.
  • Serves as a preceptor and mentor.
  • Adheres to Attendance Standards.
  • Demonstrates knowledge of department-specific billing policies and application of their procedures, including Facility Only Coding and nurse visits.
  • Demonstrates ability to use computer applications required for successful completion of job requirements, including EPIC, Transplant database, UNET, and other UWMC programs.
  • Assumes responsibility for continued professional growth and sharing expertise with other team members.
  • Teaches other health care team member(s), using informal and formal methods.
  • Shares expertise and information formally and informally, such as publication, presentation, or community activities.
  • Participates in professional organizations and/or is responsible to develop or maintain own professional growth.
  • Participates in staff meetings in person and/or actively seeks to keep self-informed of information.
