Accountabilities Demonstrates the use of the nursing process as a problem-solving model, utilizing their knowledge and experience to anticipate and plan to meet patient and family needs at the time of admission, transfer, and discharge. Reviews all orders on new admissions, transfers, and discharges. Communicates patient changes and condition with the assigned team members as indicated Communicates with patient/family within 24 to 72 hours of admission. Attends daily interdisciplinary team rounds. Collaborates with Primary RN, case managers/social services during the patient's stay, Assumes a proactive role in coordinating interdisciplinary discharge planning for a specific patient population and facilitates access to outpatient resources that the patient may need, in collaboration with the multidisciplinary team. Works with APRNs and other Providers throughout the stay to ensure the patient is ready for discharge by noon. Demonstrates commitment to meeting the learning needs of patients and families. Utilizes appropriate resources to assist in meeting those needs and achieve positive patient outcomes Assists the patient/family in maximizing their sense of control and actively participating in their recovery. Supports the patient discharge process in terms of medication reconciliation and finalizing discharge orders, completes the discharge summary, provides patient/family education, and documents discharge teaching. Validates patient/family education and comprehension before discharge. Facilitates all follow-up appointments and ensures that ordered tests and procedures are scheduled before patients’ discharge. Communicates with patient/family 24 hours post-discharge. Conducts patient follow-up at the following intervals and as needed (3, 7, 14, and up to 30 days as needed). It serves as a liaison between the medical and surgical teams and patients and their families post-discharge. Demonstrates the knowledge and skills necessary to provide care appropriate to the age and needs of the patients served on the assigned unit. Serves as a direct care provider, as appropriate, to ensure positive outcomes. Maintains a safe and clean environment that promotes the safety of patients and their families throughout all care environments. Complies with regulatory standards, including National Patient Safety Goals. Demonstrates creativity and innovation in approach to complex problem solving. Maintains a professional appearance and adheres to the proper dress code. Always wear the hospital ID badge and locator, and promote the standard of appropriate identification for peers and staff. Attends meetings as required Serves as a role model and consistently displays a professional demeanor. Maintains clinical competency. Demonstrates professional independence in the practice of nursing. Consults internal/external experts when appropriate. Maintains documentation of the continuing education requirements. CPR/BLS and ACLS are required. Maintains certification. Attends all mandatory in-services and completes all computer-based modules as assigned Actively participates in shared governance councils and professional organizations. Actively supports and participates in performance improvement activities. Seeks/accepts training to learn new skills and maintain current competencies. Maintains professional certifications/licensure in active status and good standing. Practices within the scope and guidelines set forth by the Florida Board of Nursing. Attempts to resolve patient, physician, and visitor complaints and refers unresolved issues to managers. Demonstrates dependability/reliability by promptly reporting to work when scheduled. Requirements

  • CPR/BLS and ACLS are required.
