The organization is transforming diabetes care for one of Canada's fastest-growing and most diverse communities, where over 90,000 people live with diabetes. The Adult Diabetes Services team supports over 15,000 patients annually through the Diabetes Management Centre (DMC) and Centre for Complex Diabetes Care (CCDC). The Centre for Complex Diabetes Care (CCDC) provides specialized, interdisciplinary care for adults living with diabetes and multiple co-existing health conditions. It supports individuals living with Type 1 and Type 2 diabetes who may also be managing conditions such as cardiovascular disease, kidney disease, mental health challenges, and other complex medical concerns. The team works closely with each patient to identify barriers to self-management, optimize treatment plans, and improve overall health outcomes.
