About The Position

The Clinical Dietitian is responsible for the nutrition screening, assessment, monitoring, evaluation, and education of MMCI patients through an integrated approach to nutrition care. This unique split role involves working with patients across two service lines, Family Practice and Digestive Health, providing a broad and rewarding mix of clinical experiences. A significant portion of the role focuses on weight management, helping patients create sustainable lifestyle changes to meet their health goals. Requirements

  • Bachelor's Degree in Dietetics or Related Field.
  • Licensed Dietitian Nutritionist - Illinois Department of Financial and Professional Regulation (IDFPR).
  • Registered Dietitian (RD) - Commission on Dietetic Registration (CDR).
  • 1+ years of healthcare work experience.
  • Basic reading skills.
  • Basic writing skills.
  • Basic oral skills.
  • Ability to respond appropriately to customer/co-worker.
  • Interaction with a wide variety of people.
  • Maintain confidential information.
  • Ability to communicate only the facts to recipients or to decline to reveal information.
  • Ability to project a professional, friendly, helpful demeanor.
  • Intermediate computer knowledge: Ability to troubleshoot minor problems within a Windows OS, operates within a network environment, uses spreadsheet, database, word processing, and internet applications proficiently.
  • Learns new applications without difficulty and is able to aid others in immediate work area with computer questions.

Responsibilities

  • Provide individualized nutrition counseling to patients in Family Practice and Digestive Health settings.
  • Support and guide patients through weight management, including medical weight loss programs.
  • Collaborate closely with providers to create nutrition plans that align with patient diagnoses and care plans.
  • Educate patients on topics such as healthy eating patterns, digestive health, chronic disease prevention, and behavior modification.
  • Build strong, ongoing relationships with patients to help them make meaningful and lasting change.
  • Participate in program development and continuous improvement initiatives.
  • Responsible for nutritional assessment completions 100% per policy.
  • Meets charting guidelines 100% of the time determined by chart audits.
  • Provides quality medical nutrition therapy as appropriate on patient care units including necessary follows and in accordance with facility policy.
  • Performs nutrition focus physical exams as medically appropriate for the diagnosis of malnutrition.
  • Participates in the review of the Patient Satisfaction Data and contributes to the development of the action plan and implementation of the plan.
  • Works with other Nutritional Services Team members to provide nutrition therapy in a timely manner.
  • Must participate in a required rotation for weekend and holiday coverage.
  • Writes and manages TPN and PN including, labs orders and review with pharmacological recommendation and follow up as necessary.
  • Writes and manages enteral nutrition as necessary with follow ups as needed.
  • Participates and presents in rounds, education seminars, Journal Club, team meetings, etc. to gain knowledge in Nutrition Support and medical nutrition therapy as evidenced in personal portfolio.
  • Is able to accurately document assessment and MNT in the patient EMR and menu management program.
  • Maintains knowledge of regulations.
  • Analyzes performance compared to regulations and offers ways to better, efficiently comply to provide optimum nutrition care.
  • Fulfills CDR and IDPR education requirements and maintains report portfolio with education receipts for IDPH or CDR audit.
  • Develops and utilizes a variety of processes to determine patient education needs, ie consults, assessments, ` orders, communication with other staff, care paths, etc.
  • Works cooperatively with Dining Hosts in determining and providing patients' education needs.
  • Assists with in-services for Dining Hosts regarding diets.
  • Provides nutrition education to patients, family members, staff, and community groups.
  • Participates in the training of the Dietetic Students and Dietetic Interns.
  • Plans their curriculum, gives pertinent assignments, corrects and reviews assignments with interns.
  • Participates in intern Case Study Presentations, taking turn in being a preceptor.
  • Assists in doing intern's midterm and final review.
