About The Position

The Registered Dietitian implements individualized nutritional care plans for patients and develops or obtains nutrition education materials. The role counsels patients, families, significant others, and caregivers on prescribed diets, and monitors adherence and response to therapy. Working remotely on a Monday-through-Friday schedule from 8:30 a.m. to 5:00 p.m., the dietitian collaborates with the Nutrition Support Team and other care providers to support patients receiving home enteral and parenteral nutrition.

Requirements

Bachelor of Science in Nutrition or a related field.
Registered Dietitian with a license to practice in the assigned state or states.
At least two years of clinical experience.
Strong communication and organizational skills.
High self-motivation, initiative, flexibility, and ability to work under pressure.

Responsibilities

Co-develop, implement, and monitor clinical management of parenteral and enteral patients as part of the Nutrition Support Team.
Recommend nutrition interventions when necessary.
Review patient medical histories, anthropometrics, and relevant laboratory values to assess home nutrition support protocols and insurance payer requirements.
Counsel patients and caregivers on home enteral and parenteral nutrition therapy.
Assess patients’ understanding of and compliance with prescribed diet principles.
Ensure continuity of care with patients’ physicians, nurses, and nutrition support teams to provide high-quality, patient-centered care.
Participate in the Performance Improvement Program and ACHC accreditation.
Identify and promote nutrition program services to clinical and professional referral sources, and perform necessary in-service education.
Develop or obtain patient nutrition education materials.
