About The Position

Warm Hearth Village, a nonprofit community, is seeking a PRN Registered Dietitian to serve the residents of the Kroontje Health Care Center. The Kroontje Center provides care in four different areas: assisted living, memory care, skilled care, and a nursing home setting. This role will provide backup for the FT registered dietitian in coordinating the nutritional care of all Center residents by completing an initial nutritional assessment and monitoring the residents’ evolving dietary needs. Central to success in this position will be a customer-service centered focus, as you'll regularly participate in care planning meetings, offer nutritional counseling and education for residents and their family members, and interact with residents throughout the Center. Ultimately, this position will be charged with providing services in accordance with all relevant local, state, and federal regulatory standards.

Requirements

A valid and active Registered Dietitian's license in the state of Virginia.
At least one year of experience working in a long-term care or hospital setting.
Excellent written and oral communication skills.

Nice To Haves

Experience with electronic charting.
Experience with MatrixCare is especially desirable.
Familiarity with the state survey process and ongoing compliance to nutrition care regulations.

Responsibilities
