BoldAge PACE is an all-inclusive program of care, personalized to meet the individual health and well-being needs of our participants. Our approach is simple: We listen to our participants and their caregivers to truly understand their needs and desires. The Registered Dietitian assesses nutritional status of PACE participants, plans and implements appropriate nutrition interventions, and communicates this information to the Interdisciplinary Team to ensure the nutritional needs of the participants are met. This role functions as a member of the Interdisciplinary Team, providing nutrition counseling, education, and therapy to participants, and educating family and other caregivers regarding dietary issues. The dietitian conducts initial assessments and periodic reassessments, participates in the development of the plan of care, assists in the coordination of 24-hour care delivery, and regularly informs the Interdisciplinary Team (IDT) of the nutritional needs of each participant. They also provide education to other health care professionals as appropriate.
