About The Position

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. We are currently seeking a 1 FTE Registered Dietitian (RD) or Certified Diabetes Educator (CDE) who as a valuable member of the Community Health Centre team will provide individual and group diabetes education programs and work collaboratively within an interprofessional model of care team.

Requirements

Registered Dietitian, with current registration in good standing with The College of Dietitians of Ontario or Registered Healthcare Professional with a Diabetes Certification with current registration in good standing with their respective college, for example the College of Nurses of Ontario.
Experience in supporting individuals with complex chronic diseases with self-management of education skills is required.
Excellent interpersonal, communication, group facilitation and teaching skills.
Ability to work both independently and collaboratively with an interprofessional team.
Awareness of and sensitivity to health issues particularly within populations who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured).
Knowledge and proficiency in MS Office (MS Excel/MS Word) and electronic medical record (EMR) system experience.
Valid driver’s license.

Nice To Haves

Experience with adult learning principles and motivational interviewing training is an asset.

Responsibilities

Travel to outreach sites in Thunder Bay and the Kakabeka Site with a company vehicle may be required.
