About The Position

Corlann South East is seeking bright, ambitious individuals with a positive attitude towards working with people with an intellectual disability. The role of a Regional Transition & Pathways Manager (SCM2) is a management position responsible for providing strategic leadership, operational oversight, and governance for transition planning and pathway development across Corlann South East. As a Social Care Manager Grade 2 (SCM2), the post holder must maintain professional registration with CORU and adhere to the Social Care Workers Registration Board Code of Professional Conduct and Ethics. This role involves providing professional leadership, ensuring practice aligns with legislation, regulatory requirements, and social care standards. The post holder will lead and oversee regional transition planning processes, identifying, developing, and presenting service recommendations, pathway proposals, and future service needs to the Regional Services Management Team for informed decision-making, service development, and resource planning. The role is accountable for the planning, coordination, and governance of school leaver transitions and adult service pathways, ensuring seamless, person-centred, rights-based, and outcome-focused transitions for individuals moving from school and children's services into Corlann adult services.

Requirements

  • Registered in the Social Care Workers Register maintained by the Social Care Workers Registration Board maintained by CORU or eligible under the Section 91 (existing practitioner) route for registration (evidence of application status is necessary).
  • Hold a full clean driving licence and access to a vehicle.
  • Minimum of five years' post-qualification experience in disability services.
  • Significant leadership, supervisory or management responsibilities in disability services.

Responsibilities

  • Provide strategic leadership, operational oversight, and governance in relation to transition planning and pathway development.
  • Maintain professional registration with CORU and practice in accordance with the Social Care Workers Registration Board Code of Professional Conduct and Ethics.
  • Provide professional leadership and ensure practice is informed by relevant legislation, regulatory requirements, and recognised standards of social care practice.
  • Provide leadership and oversight of regional transition planning processes.
  • Ensure that service recommendations, pathway proposals, and future service requirements are identified, developed, and presented to the Regional Services Management Team.
  • Support informed decision-making, service development, and resource planning.
  • Have overall responsibility for the planning, coordination, and governance of school leaver transitions and adult service pathways.
  • Ensure that individuals moving from school and children's services into Corlann adult services experience seamless, person-centred, rights-based and outcome-focused transitions.
