About The Position

A unit-based staff expert, with knowledge of a specific patient population who is responsible for the coordination of care processes for patients with ST elevation myocardial infarctions (STEMI). Promotes and establishes effective relationships with local and state emergency medical services (EMS) personnel for the delivery of quality care in this patient population. Will help facilitate learning experiences for staff, students, EMS personnel as well as assisting in the orientation of new staff. Proficient and knowledgeable in providing care of complex patients under the direct supervision of a credentialed physician. Will provide direct patient care as needed. This position will care for patients in the adolescent to geriatric age groups. Employee performs clinical duties in accordance with population specific guidelines and adheres to National Patient Safety Goals as outlined in policy and procedures. Will assist in the outreach to the local and regional counties to aid in NGHS STEMI program improvement.

Requirements

Current direct patient care license or a current registry in the State of Georgia.
Current BLS and ACLS certification.
High School Diploma
Four (4) years of recent clinical experience with two (2) of the years in the area of Cardiac Catheterization procedures
Good interpersonal skills
Good verbal and written communication skills
Knowledge of audiovisual equipment
Computer skills to include excel and power point presentations
Must be able to provide education in-services within the hospital system and surrounding service area
Must be highly competent in two (2) of the three (3) procedure room modalities, i.e., scrub, monitor, or circulate

Responsibilities

Regional STEMI Education and Process Improvement. Provide initial and ongoing education classes to regional EMS system and ED departments upon request. This includes basic and advanced 12 lead EKG review, ACS, STEMI treatment, Medication, Protocols, D2B, and Customized Case studies.
Maintain a good working relationship with the EMS services.
Research each STEMI case and perform an analysis of tracked times, i.e., Door time, door to EKG time, EKG to CCL arrival time, CCL to balloon time, total D2B time. These data points may vary with current needs. The cases should be reviewed within 7 days of the STEMI arrival date. This should be done in collaboration with the PI chart abstractor.
For each case that falls outside a 90-minute D2B time, research is conducted to see if there is a documented reason for the delay according to the definitions set by CMS. The research should show where the greatest delay occurred. The data will be reported to the Cath Lab management team and Medical Director. The performing physician will be notified of the results. The final monthly report is presented at the AMI meeting.
Compose an individual letter to each EMS Service and Emergency department for each STEMI patient sent to NGHS within a given month. Each letter should contain the DOS, diagnosis, culprit vessel, blockage percent, the emergency treatment provided, the outcome of the procedure, and the overall D2B time.
Chair the AMI Process Committee.
Serve on planning committee for Annual Regional STEMI Summit and be a presenter for the meeting.
Provides continuing education for the Cardiac Catheterization Lab staff, as well as Regional EMS and Regional Hospital personnel.
Collaborates with management and staff to provide ongoing cardiac care in-services both infield and institutional, which includes basic and advanced 12 lead EKG review, ACS, STEMI treatment, Medication, Protocols, D2B, and Customized Case studies.
Identifies key personnel able to assist with Cross-Training and ongoing in-servicing.
Maintains knowledge of current research and incorporates research findings into STEMI program.
Serves as a role model by demonstrating high ethical standards and clinical/professional expertise.
Acts rapidly and assumes leadership in acute or unusual situations according to hospital departmental procedures.
Collaborates with Unit Manager, Director, and physicians concerning patient care and/or unit needs relating to STEMI patients.
Identifies and/or participates in the implementation of changes with a supportive attitude.
Functions as a clinical resource in cardiac catheterization, AMI, PCI, STEMI procedures and other unit procedures as requested.
Participates in personal, professional growth and development activities and continuing education programs.
Participates in activities of professional organizations.
Supports quality assurance activities as demonstrated by compliance with established PI standards/program and assistance with monitoring / conducting chart reviews.
Identifying / Reconciling AMI / PCI / STEMI care issues.
Actively works in the procedure room on a regular basis to maintain skill level.
Participates in the call rotation.
Contributes to the overall effectiveness of the Department and continuity of the employees' orientation progress.
Meets as needed with Critical Care Educators regarding care of STEMI patients.
Serves on at least one appropriate hospital or departmental committee and/or council. This would be in addition to the M&M meetings and AMI process team.
Performs other duties as assigned by manager.
When requested, coordinates quarterly cardiac and/or peripheral vascular M&M meetings with the assistance of the manager and the Medical Director.
Keeps an active list of potential cases for presentation by the physicians relevant to STEMI outcomes.
When requested, sends out the invitations to the physicians and appropriate staff for M&M meeting, this includes reminders to the physicians regarding the meeting.
When requested, reserves the meeting room, A/V equipment and catering for the meeting.
Assures the cases can be viewed on the A/V system and record minutes for review.
Under the supervision of a credentialed physician, assists in cardiovascular lab procedures while maintaining a sterile field.
Performs various procedures that require independent judgment and high level of skill proficiency.
Assists with care of critically ill patients.
Ensures that correct patient data is entered into digital computer and that correct technical factors for each patient are selected to achieve optimal picture quality.
Preps the procedure room and equipment for procedure to be performed, to include cleaning, removal of trash, and restocking of room between cases.
Responsible for maintaining sterile technique throughout invasive procedures.
Will be fully prepared for procedure in terms of sterile supplies required.
Ensures patients are prepped and draped appropriately, maintaining privacy and confidentiality.
Will assist the physician during cardiovascular cases through knowledge of selected equipment, selection and care of catheters, guidewires, stents, and other necessary supplies.
After training, may deploy vascular closure devices using proper sterile technique to obtain hemostasis.
Has the ability to perform and rotate through functions of scrubbing, monitoring, and circulating positions.
Under physician supervision, sets up, operates, and troubleshoots fluoroscopy and digital radiological equipment.
Operates the hemodynamic monitoring system pre-, intra-, and post- procedure with proper clinical documentation.
Properly exports data into patient's electronic health record.
Prepares and operates specialized equipment found in the Cath lab environment.
Performs and documents daily operational functions on procedure room equipment on designated documentation form.
