About The Position

Community Options, Inc. is a national non-profit agency providing residential group home, day habilitation and supported employment services to individuals with intellectual and developmental disabilities in 12 states. The Regional RN Clinical Manager provides clinical leadership, oversight, and support to Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) across multiple service regions. This role ensures the delivery of high-quality, person-centered nursing care for individuals with intellectual and developmental disabilities while maintaining compliance with regulatory standards and organizational policies. The Regional RN Clinical Manager travels regularly to provide on-site clinical support, training, supervision, and direct care as needed. The RN Clinical Manager can be based out of any NJ office and will require travel 2-3 days per week.

Requirements

  • Bachelor’s degree in nursing (BSN) or equivalent clinical nursing experience
  • Minimum of five (5) years of supervisory or management experience overseeing clinical staff (RNs and/or LPNs)
  • Minimum of one (1) year of experience providing nursing services to individuals with intellectual and developmental disabilities
  • Current, valid, unrestricted nursing license in the state of employment
  • Valid driver’s license with a satisfactory driving record and ability to travel regularly
  • Strong commitment to community-based services for individuals with disabilities
  • Excellent verbal and written communication skills
  • Successful completion of all required background checks and drug screening

Responsibilities

  • Provide clinical supervision, guidance, and performance support to RNs and LPNs within assigned regions
  • Conduct regular regional visits to observe nursing practices, mentor staff, and promote consistent quality of care
  • Serve as a clinical resource for complex cases, care planning, and problem-solving
  • Ensure nursing staff comply with professional standards of practice, physician orders, and regulatory requirements
  • Provide nursing treatments and health care procedures as ordered by a physician and required by professional standards or law when necessary
  • Conduct nursing assessments and follow-up visits
  • Monitor individual health conditions and ensure timely medical referrals
  • Administer medications and provide medication management when required
  • Provide health education and support individuals in developing self-care and safety skills
  • Ensure individuals attend medical appointments in accordance with their Individual Support Plans (ISP)
  • Develop, update, and participate in interdisciplinary ISP and Person-Centered Plan (PCP) meetings
  • Document medical concerns, observations, and behavioral developments
  • Prepare and distribute medical and health reports summarizing overall health status, goals, and physician recommendations
  • Ensure accurate transcription and maintenance of all physician orders and medical documentation
  • Conduct programmatic trainings, including medication administration, delegation, and health related competencies
  • Provide new employee orientation, annual recertification, and ongoing competency validation
  • Train staff on the medical needs of individuals served and ensure required competencies are met
  • Complete supervisory visits in accordance with regulatory and organizational standard
  • Review care plans, caregiver competencies, and nursing documentation
  • Ensure timely completion of daily, monthly, quarterly, and annual nursing documentation
  • Report all incidents in accordance with operational and regulatory procedures
  • Maintain compliance with all operational, state, and federal regulations
  • Cooperate fully with licensing agencies and regulatory authorities during inspections or investigations
  • Foster strong collaboration across interdisciplinary teams and regional staff
