The Registered Dietitian Diabetes Educator is responsible for screening and assessing the nutritional status of patients/clients seeking diabetes education. This role involves collaborating with patients to develop and implement personalized care plans, documenting all relevant information in the electronic medical record, and coordinating services with the diabetes management team and ancillary services. The educator will train patients on blood glucose monitoring, including continuous glucose sensors (CGMS) and insulin pumps, as well as the use of prescribed medications, including injectables. This position also serves as a diabetes education care resource person for the system and the diabetes program, participating in ongoing program development, policy/procedure updates, and quality initiatives related to diabetes care, clinical development, and efficiency. The role requires accurate charge entry into EPIC and providing education to staff members on diabetes care, medical nutrition therapy, and program materials. Maintaining required continuing education units (CEUs/CMEs) for licensure/registration/certification is essential, along with attending mandatory in-services and online education. Participation in program needs, peer review, and familiarity with ADA program requirements are also expected.
