About The Position

Under direct supervision of the Diabetes/ Wellness Manager, this position is responsible for planning, organizing, and directing program and activities focused on health promotion through evidence-based knowledge of diabetes-related education and clinical nutrition. The role involves providing diabetes-related education and nutrition counseling to individuals with Type 1 and Type 2 diabetes, gestational diabetes, pre-diabetes, and those with family histories. The Diabetes Educator will function as a collaborative member of the Diabetes Team for routine care and contribute to ongoing health and wellness activities.

Requirements

  • Clinical experience working with people with diabetes, pre diabetes, cardiovascular disease, obesity, etc. required.
  • Demonstrated ability to communicate and work effectively with Native Americans.
  • Must be able to work in a diverse minority environment.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to represent the organization in a professional manner, building respect and confidence.
  • Ability to handle multiple tasks and meet deadlines.
  • Ability to carry out instructions furnished in verbal or written format.
  • Ability to work independently with minimal supervision.
  • Skill in operating business computers and office machines, including Windows (i.e. Word, Excel, PowerPoint etc.).
  • Negative alcohol and drug screen.
  • Meet requirements of Public Law 101-630.

Nice To Haves

  • Certified Diabetes Educator (CDE) preferred.
  • Master’s degree (preferred) or bachelor’s degree in nursing or nutrition from an accredited college or university.
  • Experience in clinical and community nutrition strongly preferred.

Responsibilities

  • Plans, organizes, and directs education and nutrition services for the Health Center.
  • Provides nutrition support for the Health Care Team, including Community Health.
  • Identifies, develops, and distributes diabetes-related and nutritional education materials for the Health Center and all communities.
  • Provides diabetes and nutritional education to meet prevention and treatment needs for individuals and specific groups (e.g., pre-natal, youth, diabetic, pre-diabetic, obese).
  • Provides alternative food preparation methods to specific groups, including the Commodity Food Program and others upon request.
  • Plans for and administers the Summer Foods Program in accordance with SFSP regulations.
  • Assists Senior Program staff in menu development and certifies menus.
  • Provides referred clients with an evaluation and objective-based nutritional care plan, including individual counseling.
  • Assists in developing the budget for the diabetes education and nutrition program.
  • Contributes to monthly, quarterly, and annual reports.
  • Performs other duties as assigned.
