About The Position

Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area. This role provides quality education, counsel, and support to clients and their significant others regarding diabetes self-care. The position involves documenting the education process, patient information and clinical recommendations according to department/program/regulatory guidelines, and measuring outcomes to maximize continuity of care between settings. Participation in projects related to the area of specialty and the development/revision of procedures/policies, educational materials/guidelines, and educational programs is expected. The role also involves collaborating with and serving as a resource for other health professionals to promote the delivery of quality services and patient education within the institution as well as the community.

Requirements

• Bachelor's Degree Nursing or related healthcare field
• LIC-Registered Nurse (RN) - STATE_MI State of Michigan required or CRT-Registered Dietitian (RD) - CDR Commission on Dietetic Registration required
• CRT-CPR - AHA American Heart Association 90 Days required
• CRT-Diabetes Educator (CDE) - NCBDE National Certification Board for Diabetes Educators 3 Years required

Nice To Haves

• Master's Degree
• 3 years of recent, relevant clinical experience including prior teaching experience with adult clientele
